Privately Sponsored Travel: Traveler Form

This form should be submitted to the Committee along with the Private Sponsor Certification form no later than 14 days before the start of the trip.  You must keep a copy of all forms and documents you submit to the Committee

Before submitting your travel request, please be sure:

1. The first page of the request is signed by the traveler or designee
2. For staff, the second page is signed by your employing Member
3. You have attached a completed Privately Sponsored Certification Form and all required materials
